Private Pay New Consultation

Initial specialist consultation for non-OHIP or uninsured services.

Private Pay Follow-Up Appointment

$125

Follow-up specialist visit for non-OHIP or uninsured services.

Prior Authorization Form

$50

Completion of insurance or drug plan prior authorization paperwork.

Doctor Note

$30

Physician note for work, school, or other purposes.

Drivers Medical Form

$75

Completion of a Ministry of Transportation drivers medical form.

Attending Physician Statement

$125

Completion of an attending physician statement for insurance purposes.

Disability Tax Credit Form

$125

Completion of the CRA Disability Tax Credit Certificate (T2201).

Missed Appointment Fee

$100

Fee applied when an appointment is missed without 24 hours notice.

• Missed appointment fees apply when cancellation is not received at least 24 hours in advance.
